Important Information for All Patients

📋 Insurance Coverage

We work with a variety of insurance plans and are actively expanding our network. To find out which insurances are currently accepted, please email us at:

Some plans, including Medicare, are in progress and may be added soon.
It is the patient’s responsibility to ensure that a current and valid referral from your referring physician is on file prior to starting or continuing care.

🧾 Private Pay / No Insurance

If you do not have active insurance coverage, full payment is required at the time of service.

📞 Cancellation & No-Show Policy

If you are unable to attend your scheduled appointment, please call to cancel as soon as possible.

  • Cancellations must be made at least 24 hours in advance

  • No-shows or late cancellations will result in a $50 fee, which is not billable to insurance.

We understand that life happens—please contact us if there are extenuating circumstances.

💳 Payment & Past Due Balances

  • You will receive a statement for any balance after your insurance has processed the claim.

  • Balances are due upon receipt.

  • If you are experiencing financial hardship, please call our billing department to discuss flexible payment options.

Please note: If your balance remains unpaid despite repeated reminders, we may be required to take necessary collection or legal action. Our priority is to work with you compassionately to avoid this outcome.
